Population Health

In 2016, Sparrow Health System, Physicians Health Plan, and Sparrow Care Network began collaborating to improve the health of our entire region. Through Sparrow’s Population Health initiative, time and resources are being invested to improve the quality and affordability of the care we deliver, and the experience we provide to our Patients.

Initial work began in May 2016 with the creation of a Congestive Heart Failure Care Collaborative Workgroup (CCW). Using Mayo Clinic Best Practice Guidelines as the initial framework, the Sparrow-specific Congestive Heart Failure Best Practice was finalized in May 2017 and was featured in Epic’s Clinical Program Honor Roll, which highlights areas where electronic medical records (EMR) are used to advance quality initiatives.

Building off this success, Sparrow has initiated CCWs for chronic obstructive pulmonary disease (COPD), diabetes, and cardiac care. Care transformation through CCWs will continue to be a priority for our Population Health initiative in 2018. Other 2018 priorities will focus on improving care transitions through post-acute provider relationships and developing the capabilities necessary to address the needs of our complex Patient population.
